Randolph Hockey Club Registration

The following forms must be completed and signed for registration:

1. Registration (to be signed by player and parent)

2. Consent to Treat / Medical History

3. Code Of Conduct (to be signed by player)

PLEASE NOTE: All players must register themselves for USA Hockey. Click here to register.

  A PLAYER CANNOT PRACTICE OR PLAY UNLESS THEY ARE REGISTERED WITH USA HOCKEY !

To expedite registration, please ensure you have all the forms and they are completely filled out and signed.

If you cannot attend the signup sessions, please drop the forms off at the rec center by June 30th, or contact one of the coaches or committee members.
